Scope

YY/T 0285.6-2020 is the English-translated version of 血管内导管 一次性使用无菌导管 第6部分:皮下植入式给药装置.

YY/T 0285.6-2020 is Part 6 of the Chinese standard series on sterile single-use intravascular catheters, and covers subcutaneous implanted ports - the infusion port placed under the skin and connected to a catheter, used for long-term chemotherapy, parenteral nutrition and repeated drug delivery.

A port is punctured hundreds of times over months or years and must neither leak nor shed septum debris into the bloodstream, so the standard sets the performance and safety requirements a manufacturer has to demonstrate.

It covers biocompatibility and bacterial endotoxin, chemical properties of silicone and other components, ethylene oxide residue, distance marks, nominal dimensions, gas tightness, self-sealing after repeated puncture, puncture chips, peak tension, bending and dynamic breaking strength of the connection, flow for ports used and not used for power injection, burst pressure, MRI compatibility and the information supplied by the manufacturer.

It relates to ISO 10555-6:2015 (NEQ); requirements for the non-coring needle are in YY 0881. Issued by the National Medical Products Administration on 27 September 2020, it has applied since 1 September 2021.

4.6.2 Ports indicated not to be used for power injection: the test of Annex E of YY 0285.1-2017 is carried out with the non-coring needle connected to the Luer connector, placed through the septum, the port connected to the test catheter, and the in-line pressure sensor at the proximal end of the needle. Results are expressed as the minimum and maximum gravity flow; the needle gauge and length are recorded.

4.6.3 Ports indicated to be used for power injection: the test of Annex G of YY 0285.1-2017 is carried out with the locking device connected to the hub of the power-injection non-coring needle, the needle placed through the septum and the port connected to the test catheter.

4.7 Burst pressure for subcutaneous implanted ports and catheters

4.7.1 General: the test is carried out by the method of Annex F of YY 0285.1-2017, with the locking device accessories connected to the hub of the non-coring needle as justified by a risk-based clinical rationale.

A Annex A (normative) Test method for no gas leakage

A.1 Principle: air pressure is applied to the port and to the connection between port and catheter, and the assembly is checked for gas leakage.

A.2 Apparatus: leak-proof connectors; syringe or constant pressure source; pressure gauge capable of measuring up to 300 kPa; temperature-controlled test chamber that can be filled with liquid.

A.3 Procedure: bring the chamber liquid to (37 ± 2) °C and maintain it; connect a split catheter to the port; cut the catheter leaving up to 10 cm for connection to the gauge and pressure source; connect through a leak-proof connector; immerse the port and allow it to reach thermal equilibrium; raise the pressure to at least 200 kPa and wait 2 min.

A.4 A gas leak is a pressure drop of more than 2.65 kPa within 2 min, or failure to reach 200 kPa.

D Annex D (normative) Test method for no leakage after multiple punctures

D.1 Principle: the effective area of the septum is punctured the appropriate number of times at random positions with the non-coring needle, and the port is then leak tested according to Annex A.

D.3 Procedure: at least 1,000 punctures per cm2 of effective septum area are made, or the higher number claimed by the manufacturer, with the port kept at (37 ± 2) °C. Each needle is used for at most 50 punctures and is replaced if its tip is bent or wobbles after leaving the septum. The leak test of Annex A is then carried out.
