1 Scope

This standard specifies the data structure, data element attributes, data format and data management requirements of personal health information code data.

This standard is applicable to the design, development and system integration of personal health information related application systems, as well as the collection and exchange of personal health information code data.

3 Term and definition

For the purposes of this document, the terms and definitions given in GB/T 38961- 2020 and the following apply.

4 Personal health information code data structure

Personal health information code data includes:

a) Personal basic information: the identity information of the subject (natural person) of personal health information. Including name, gender, nationality, ID type, ID number, administrative division of household registration, detailed address in the divisions, contact number, basic medical history, etc.

b) Personal health information: the health data of the subject of personal health information (natural person). Including body temperature, current symptoms, living and staying conditions in high-risk areas, exposure to high-risk personnel, and reporting time of health information, etc.

c) Travel information: the location information of the subject of personal health information (natural person) who has lived or stayed for more than a certain period of time in the recent period. Including the provinces (autonomous regions, municipalities directly under the Central Government, special administrative regions, Xinjiang Production and Construction Corps, etc.), cities (states, leagues, etc.), districts (counties, county-level cities, banners, etc.) where they are currently located or have stayed, and the verification of travel information, etc.

d) Health certificate information: the health status evaluation result of the subject (natural person) of personal health information by the personal health information management department. Including health risk level, time and reason of health risk level assessment, medical test results, medical testing organizations, medical test time and data sources.
